Middle Childhood Education Information

Throughout the program, supervisors will need to access documents on this page to support, monitor, and assess candidate growth. Supervisors can also reference the Student Information and Mentor Information pages from the menu on the left side of the page. Supervisors will need to maintain records and complete documentation throughout each term.

All Middle Childhood Education students are required to be read and understand the Program Handbook. 

The Middle Childhood Program has also developed a Supervisor Handbook containing the policies and procedures specific to supervisors.

Professional Cohort Year 1

During Spring Semester candidates in their first year of professional cohort participate in a field experience known as Practicum I.

Spring Semester Supervision - Practicum 1

Due: If Needed

A supervisor can complete if a concern emerges, an incident occurs, or if the candidate is exemplary in some way. If something positive or negative occurs about which the field coordinator should be informed, please complete this form

This survey is critical to ensuring quality placements. The survey will ask you to rate your experiences the mentor teacher and placement school. This information is used for the purpose of strengthening our placement processes. This form will be due by the end of the semester.

Supervisors are required to document their communication with candidates and mentors throughout the entire term.

Professional Cohort Year 2

During the final year of professional cohort candidates participate in two field experiences: Practicum 2 and Student Teaching. The candidate will be placed with the same mentor teacher for both experiences.

This observation form is used by both university supervisors and mentor teachers to provide written feedback to candidates on observed instruction. Submit online twice during the semester. Email copy to candidate and mentor

This document is completed independently by the candidate, the mentor teacher, and the university supervisor. All three parties bring their completed assessments to a triad meeting to reach consensus and develop goals for Student Teaching, Supervisor is responsible for online submission.

This observation form is used by both university supervisors and mentor teachers to provide written feedback to candidates on observed instruction. Not required to be submitted online. Give the candidate a copy (and supervisor, if desired.) No set number of observations is required from the mentor.
