Middle Childhood Mentor Information

Welcome Mentor Teachers!

Throughout the program, mentor teachers will need to access documents on this page to support, monitor, and assess candidate growth. Mentor teachers can also reference the Student Information and Supervisor Information pages from the menu on the left side of the page.

All Middle Childhood education students are required to be read and understand the Program Handbook, and keep a copy in their binder. This handbook outlines the policies and procedures of the program.

Professional Cohort Year 1 - Practicum 1

During Spring Semester candidates in their first year of professional cohort participate in a field experience known as Practicum I.

April 10 Mentor Teacher Evaluation of Program Feedback to UC about the student teaching mentoring experience. Information is used for both accreditation purposes as well as for program improvement.

Professional Cohort Year 2 - Senior Year

During the final year of professional cohort candidates participate in two field experiences: Practicum 2 and Student Teaching. The candidate will be placed with the same mentor teacher for both experiences.

The chart below identifies the forms mentors are responsible for completing during Practicum 2 and Student Teaching.

AMLE and Ohio Standards of the Teaching Profession Observation Tool (DOC)

AMLE Qualtrics Link

This observation form is used by both university supervisors and mentor teachers to provide written feedback to candidates on observed instruction. Not required to be submitted online. Give the candidate a copy (and supervisor, if desired.) No set number of observations is required from the mentor.
Every other week Collaborative Assessment Log (CAL) (DOC)
Complete in a conference with teacher candidate.  The student teacher is required to fill out the form and keep in their binder.
